Bridging the Gap of Van Zandt County serves seniors and in-home health patients who face unique challenges due to age, illness, or disability. Our focus is on individuals who are in need of resources and assistance that fall outside the scope of traditional healthcare coverage, such as mobility aids, home safety modifications, or other personal items essential for daily living. We particularly target those who may not have access to these critical resources due to financial constraints, lack of support, or gaps in insurance programs. By providing this vital assistance, we ensure that vulnerable community members—many of whom live on fixed incomes—are able to maintain their independence and quality of life, regardless of their circumstances.
Seniors on Fixed Incomes

Bridging the Gap of Van Zandt County is a 501c3 whose mission is to provide community resources for in home health patients & other at need seniors who have personal needs outside of those covered by healthcare programs.They literally bridge the gap by filling in with assistance for items and resources not covered by these programs.
